Pediatric to Adult Care Transitions: Between and Across Settings
This activity addresses the complex challenges, barriers and optimal successes in transitioning childhood palliative care patients and families over to adult palliative care and adult services providers. The desired outcome for this activity is to apply, analyze and evaluate a transition model in pediatric to adult transitions for future practice application to improve outcomes in pediatric to adult serious illness care.
