Elevating Patient and Caregiver Voices: Implementing New Patient-Reported Experience Measures

This activity is designed to facilitate implementation of PRO-PMs to improve quality outcomes in serious illness care. 
1. It is imperative that we focus on the aspects of the healthcare delivery experience that are the most important to patients and families receiving palliative care services with the aim to improve quality of care. Two new patient-reported experience measures are available at no charge from AAHPM, as well as an Implementation Guide for practice application. The new measures assess the quality of care provided by asking (1) how much patients felt heard and understood, and (2) if patients got the help they wanted for their pain. 
2. This webinar will introduce the new palliative care patient-reported outcome performance measures (PRO-PMs) and go through examples from diverse members of the interdisciplinary (IDT) team about how they’re linking the measures to quality improvement. Learn about patient-centered quality measurement in practice, generate ideas for overcoming barriers to measure implementation including leadership support, data collection, and workflow, and explore how the new measures can be used to help patients and families feel more engaged and included in their care.
3. Participants will learn about the steps to implementing these measures in practice and how to use the Implementation Guide within individual practice settings.
