Learning Outcomes

Using a high-level framework and a "way of thinking" surrounding goal-concordant care, learners will adapt hospice describing clinical practices to:

  1. Facilitate a multiple-step, person-centered approach that includes comprehensive assessment of the individual’s clinical status, diagnosis-specific status, and medications with goals of care and shared decision-making.
  2. Incorporate a step-wise approach in describing to include medication, life expectancy, goals of care, and whether a medication is likely to achieve goals or targets.

The American Academy of Hospice and Palliative Medicine (AAHPM) designates this Enduring material for a maximum of 1.0 AMA PRA Category 1 CreditsTM. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

CME credits expire May 31, 2025.

Maintenance of Certification (MOC)

Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 1.0 Medical Knowledge MOC points in the American Board of Internal Medicine's (ABIM) Maintenance of Certification (MOC) program. It is the CME activity provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.
